1. Agreement to Terms
By accessing or using Heal from Home's services, website, or booking a telehealth session, you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, please do not use our services.
2. Services Provided
Heal from Home provides telehealth physical therapy services via video consultation (Zoom). Our services include:
- Initial evaluations and assessments
- Follow-up treatment sessions
- Multi-session therapy packages
All services are provided by licensed physical therapists operating under the PT Compact, authorized to practice in 37+ states.
3. Cash-Based Practice
Heal from Home is a cash-based (private pay) practice. We do not bill insurance companies or accept insurance as a form of payment.
- All fees are due at the time of booking
- Payment is processed securely through Stripe
- We can provide a superbill (itemized receipt) upon request, which you may submit to your insurance for potential out-of-network reimbursement — however, reimbursement is not guaranteed
- We are not responsible for insurance reimbursement claims or denials
4. Booking & Payment
- Sessions are booked through our website and confirmed after successful payment
- Payment is required at the time of booking to secure your appointment
- All prices are displayed in US dollars and include applicable taxes
- Package sessions (e.g., 4-Session Package) are valid for 90 days from purchase date
5. Cancellation & Refund Policy
24-Hour Cancellation Policy
- More than 24 hours before your session: Full refund or free rescheduling
- Less than 24 hours before your session: The session fee will be charged in full. You may reschedule (subject to availability) but no refund will be issued
- No-shows: The full session fee will be charged with no refund or reschedule
- Package sessions: Cancellation policy applies to each individual session. Unused package sessions are non-refundable after 90 days

6. Telehealth Consent
By booking a session, you acknowledge and agree to the following:
Nature of Telehealth
Telehealth involves receiving physical therapy services remotely through video technology. Your therapist will evaluate, diagnose, and treat your condition via live video consultation rather than an in-person visit.
Benefits of Telehealth
- Access to expert physical therapy from your home or any private location
- No commute time or travel expenses
- Ability to receive care across state lines (via PT Compact licensure)
- Flexible scheduling for your convenience
Risks & Limitations
- The therapist cannot physically touch or manually assess you — some conditions may require in-person evaluation
- Technical difficulties (internet disruptions, audio/video issues) may interrupt or delay your session
- There is a small risk that information could be intercepted during electronic transmission, despite encryption safeguards
- Telehealth may not be appropriate for all conditions — your therapist will advise if in-person care is recommended
Technology Requirements
- A device with a camera and microphone (smartphone, tablet, or computer)
- Stable internet connection (broadband or strong Wi-Fi recommended)
- Zoom application (free to download) or a compatible web browser
- A private, well-lit area with approximately 6 feet of space to move
Emergency Protocols
Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
If you experience sudden severe pain, dizziness, difficulty breathing, or any other concerning symptoms during a session, inform your therapist immediately. Your therapist will help you determine the appropriate course of action, which may include ending the session and seeking in-person care.
Your Consent
By completing your booking, you confirm that you:
- Understand the nature, benefits, and risks of telehealth physical therapy
- Consent to receive treatment via telehealth technology
- Understand that you may withdraw consent at any time
- Have the right to request in-person referral if telehealth is not suitable for your needs
7. Patient Responsibilities
- Provide accurate and complete health information
- Follow your therapist's instructions during sessions
- Perform at-home exercises as prescribed between sessions
- Notify your therapist of any changes in your condition
- Ensure a safe environment for performing exercises during telehealth sessions
- Join sessions on time with a working camera and microphone
8. Limitation of Liability
To the fullest extent permitted by law:
- Heal from Home is not liable for injuries that may occur while performing exercises at home, including those prescribed during sessions
- We are not responsible for technical failures, internet disruptions, or third-party platform issues (Zoom, Stripe) that may affect your service
- Our total liability for any claim arising from our services shall not exceed the amount you paid for the specific session in question
- We do not guarantee specific health outcomes — results vary based on individual conditions, compliance, and other factors
9. Intellectual Property
All content on the Heal from Home website — including text, images, logos, exercise programs, and educational materials — is owned by Heal from Home and protected by copyright law. You may not reproduce, distribute, or create derivative works without our written permission.
10. Governing Law
These Terms are governed by the laws of the State of New Jersey, without regard to conflict of law principles. Any disputes arising from these Terms or our services shall be resolved in the courts of New Jersey.
11. Changes to Terms
We may update these Terms from time to time. Changes will be posted on this page with an updated "Last updated" date. Your continued use of our services after changes constitutes acceptance of the updated Terms.
